In the largest prison in Brazil, Carandiru, a medical doctor named Drauzio Varella begins an AIDS prevention program, despite the prison being overcrowded by nearly 2,000 inmates. As he navigates the complex lives of the prisoners, he encounters a mix of hardened criminals and otherwise decent individuals who have been driven to desperate acts. Just as the doctor is making progress, a seemingly trivial argument over clothing leads to a violent uprising, which is brutally suppressed by the authorities.
