In the heart of Calcutta, two families from different walks of life converge in the City of Joy, a slum neighborhood. The Pal family, who lost their farm, is seeking a better life, while Max Lowe, a surgeon from Houston, has come to find spiritual enlightenment. They meet at the City of Joy Clinic and School, run by Joy Bethel, a compassionate Irishwoman. As they settle in, Joy convinces Max to provide medical services, which he reluctantly agrees to. However, their peaceful existence is threatened by the corrupt Ghatak family, who own much of the neighborhood and demand "protection money" from businesses. The clinic's decision to help a nearby leper colony also sparks resistance from some neighbors.
