In the inner city of Los Angeles, two rival gangs, Los Locos and La 213, are led by Loco and Riot. When Loco's younger brother ventures into La 213's territory, tensions escalate and the two gangs clash in a violent showdown. Loco and his girlfriend Mia, along with other gang members, flee to an abandoned hospital, where they must confront a deranged drug dealer named Tweaker and prepare for Riot's imminent return.
