Fourteen years after the brutal murder of Dr. Karl Matthews by the ancient soul-taker Mr. Hell, his daughter Tyler returns to the scene of the crime. Unbeknownst to her, Mr. Hell has also resurfaced. Adding to the chaos, a group of mercenaries invades the lab to steal a deadly bacterium, leading to a series of gruesome deaths and eye harvests at the hands of Mr. Hell.
