In this dark comedy, a notorious Baltimore criminal and underground figure, Divine, clashes with a morally corrupt married couple who attempt to degrade and humiliate her, as they vie for the title of "The Filthiest Person Alive".

Divine, a notorious figure, resides in a caravan with her eccentric son Crackers and her 250-pound mother Mama Edie, attempting to enjoy their reputation as the "filthiest people alive". However, they are threatened by Connie and Raymond Marble, a notorious duo who peddle heroin to schoolchildren, kidnap and impregnate female hitchhikers, and sell the babies to lesbian couples. The Marbles eventually challenge Divine, sparking a fierce confrontation.
